Understanding GLP-1 Medications (Semaglutide & Tirzepatide)

GLP-1 receptor agonists work by mimicking gut hormones that signal fullness to the brain, slow stomach emptying, and reduce appetite. They do not change the anatomy of the stomach or intestines.

In clinical trials, GLP-1 medications produce meaningful weight loss typically 15 to 21% of total body weight. For many patients, that is a significant and life-changing result. But there are important limitations that patients need to understand before choosing this path.

The Durability Problem

GLP-1 medications only work while you are taking them. When patients stop, the weight comes back. A 2025 systematic review and meta-analysis (Berg et al., Obesity Reviews) found that patients stopping semaglutide or tirzepatide regained a mean of 9.69 kg, with regain proportional to how much was originally lost. University of Oxford research confirms that most patients return to their pre-treatment weight within approximately 18 months of stopping.

The Discontinuation Reality

According to data from the ASMBS 2025 Annual Scientific Meeting, up to 70% of patients stop taking GLP-1 medications within one year which is most commonly due to cost, insurance coverage gaps, or gastrointestinal side effects. This makes the real-world durability of GLP-1 therapy considerably lower than clinical trial numbers suggest.

The Long-Term Cost

GLP-1 medications are an ongoing monthly expense. A 2025 study published in JAMA Surgery (Barrett et al.) found that over two years, patients on GLP-1 therapy spent approximately $11,689 more than patients who had bariatric surgery.. For patients who take GLP-1 medications for five, ten, or twenty years, the cumulative expense is substantial.

When GLP-1 Medications Make Sense

GLP-1 medications are a legitimate and effective option for many patients  particularly those who are not yet candidates for surgery, prefer a non-surgical approach, or want to lose weight before surgery to reduce operative risk. Understanding Gastric Sleeve Surgery

Gastric sleeve surgery permanently removes approximately 75 to 80% of the stomach, leaving a narrow, tube-shaped pouch. Unlike GLP-1 medications, gastric sleeve surgery creates a structural change that does not require ongoing treatment to maintain its effect.

Gastric sleeve surgery produces approximately 60 to 70% excess body weight loss in the first 12 to 18 months, with durable long-term results. It also improves or resolves many obesity-related conditions including type 2 diabetes, high blood pressure, sleep apnea, and joint pain.

Who Sleeve Surgery Is Best For

Gastric sleeve is generally best suited for patients with a BMI in the 30 to 49 range, earlier-stage metabolic disease, and a preference for a straightforward digestive anatomy after surgery. It is also a natural next step for patients who have plateaued or stopped GLP-1 medications and want a more permanent solution.

One important consideration: a small number of patients experience worsening acid reflux (GERD) after sleeve surgery because the procedure removes the fundus of the stomach, which can affect the lower esophageal sphincter. Patients with existing GERD should discuss this carefully with Dr. Bonnor during consultation.

Understanding SADI Surgery

SADI or single anastomosis duodenal-interposition is best understood as a gastric sleeve plus an intestinal bypass. It includes everything sleeve surgery does and then adds a single surgical connection between the duodenum and the lower small intestine. This intestinal bypass creates a powerful malabsorptive component that dramatically amplifies both weight loss and metabolic outcomes.

Because SADI uses a single intestinal connection rather than the two required in traditional gastric bypass, it is considered technically simpler than Roux-en-Y bypass while delivering comparable and in recent studies, superior  results. The landmark SADISLEEVE randomised controlled trial (Robert et al., The Lancet, 2025) demonstrated SADI produced superior weight loss versus Roux-en-Y gastric bypass at 2-year follow-up with a similar safety profile.

A 2025 systematic review and meta-analysis (Sabatella et al., Obesity Surgery) found SADI produced 3.48 times higher odds of type 2 diabetes remission compared to Roux-en-Y gastric bypass — a striking metabolic advantage that makes SADI particularly relevant for patients with advanced or poorly controlled diabetes.

How SADI Differs From Sleeve

The sleeve alone works on restriction and hunger hormone reduction. SADI does all of that and adds a reduction in calorie absorption from fat. Patients eat less, absorb fewer fat calories, and experience a stronger hormonal response which also includes an enhanced GLP-1 signaling from the gut itself, which drives deeper metabolic change. SADI patients typically achieve 70 to 90% excess body weight loss, with stronger long-term durability than sleeve alone.

SADI also preserves the pyloric valve which is the natural passage between the stomach and duodenum  which reduces the risk of dumping syndrome that is more commonly associated with traditional gastric bypass. This is a meaningful quality-of-life advantage.

Indications for SADI

SADI is best suited for patients with a BMI of 40 or higher, severe or poorly controlled type 2 diabetes, metabolic syndrome with multiple comorbidities, or inadequate results after a prior sleeve gastrectomy. It is also the most effective revision option available for sleeve patients who need more when the sleeve component is already in place, and the revision adds the intestinal bypass component.  In fact this is the preferred revision for weight loss.

Because SADI reduces fat absorption, long-term supplementation of fat-soluble vitamins (A, D, E, K), vitamin B12, iron, calcium, and zinc is essential. Routine lab monitoring is a non-negotiable part of long-term care after SADI.

Which Option May Be Right for You?

Consider GLP-1 Medications if you:

  • Are not yet a candidate for surgery but want to begin losing weight

  • Want to reduce surgical risk by losing weight before bariatric surgery

  • Prefer a non-surgical approach and understand the long-term commitment required

Revision bariatric surgery is more complex than primary surgery due to scar tissue and altered anatomy, which is why selecting an experienced surgeon from the start matters.
